Part 1: Complete the Health and Wellness Assessment

  1. Download and print the PDF of The Wellness Assessment
  2. For each question, write the number (1-5) that corresponds to your response to each question in the Wellness Assessment
  3. After you have responded to all 60 questions, calculate your scores for the six wellness categories (eg: Emotional Health, Fitness and Body Care etc.)

Part 2: Reaction Paper


Refer to your scores on the Wellness Assessment and write a paragraph (minimum) response to each item below.
  1. List two aspects of your personal health that you are the most pleased with and explain why you feel that way.
  2. List two aspects of your personal health that you are the least pleased with and explain why you feel that way.
  3. List three items in The Wellness Assessment that interested you and explain why.
  4. Indicate one or more aspects of your personal health that you would like to change and explain why. Consider using the Course Project as a way to make a positive health change in your life.
  5. Identify factors that prevent you from accomplishing your health goals.

Submit to the Instructor via E-mail


1. The totals for each of the six wellness categories in Part 1.
2. Your responses to Part 2.

Evaluation


1. Five points for Part 1
2. One point for each response in Part 2 (total = 5 points).
